Back-to-School Immunization Clinics







The time is now to call and schedule your child's immunizations before the school year begins on August 10, 2016.

Franklin County Public Health is offering immunizations the fourth Tuesday every month at the Reynoldsburg Police Department at 7240 E. Main St. In addition, four additional evening and weekend clinics will be offered on August 9 and 10, as well as September 10 and 20, 2016, at the Franklin County Public Health office in Columbus. See the attachment for more information

Required childhood immunizations are available to those who have no insurance for a $15 fee per shot; however, uninsured children will not be turned away due to the inability to pay. Cash or check, Medicaid, CareSource, Buckeye, Paramount, UHC Community and Molina health plans are accepted. Most major medical plans will be accepted starting in early 2016. Adult vaccines also are available.

Please remember to bring your child's shot record, a photo ID of the parent and your insurance card.

Visit myfcph.org/imm to schedule online or call (614) 525-3719 to make your appointment. Most appointments will be scheduled between 9:40 a.m. - 2 p.m. A scheduled appointment is required.
