Insurance Information
Medical Insurance
Medical and Prescription Drug Insurance is provided by Medical Mutual for all full-time certified and classified employees. A full-time classified employee is scheduled to work 40 hours per week with the exception of cooks and bus drivers who shall be considered full-time if they are assigned to work 27.5 hours and 20 hours per week, respectively.

Part-time certified employees (working less than 7.75 hours per day) will be offered Medical Insurance at a prorated share based on the number of hours contracted per day. Part-time classified employees working at least 20 hours per week will be provided with Medical Insurance at a prorated share based upon the proportion of full-time hours of the scheduled contribution rate for full-time employees.

To waive coverage, please complete and return BOTH the Enrollment form and the Waiver of Participation. Page two of the Enrollment form has a waiver section that must be completed.

Open enrollment begins in November. Coverage can be dropped at any time for any reason. Additions or changes due to a qualifying event such as marriage, birth, etc. must be made within 30 days of the event. Please complete an enrollment form and provide back-up documentation.
 
Dependents may be covered until the age of 26 is reached.
Vision Insurance
Vision Insurance is provided by Medical Mutual for full-time certified and classified employees. A full-time classified employee is scheduled to work 40 hours per week with the exception of cooks and bus drivers who shall be considered full-time if they are assigned to work 27.5 hours and twenty 20 hours per week, respectively.

Part-time certified employees (working less than 7.75 hours per day) will be offered Vision Insurance at a prorated share based on the number of hours contracted per day.

The Vision Claim Form should be taken to appointments and completed by your doctor. Attach the Claim Form with your bill and send to Medical Mutual for reimbursement.

Dependents may be covered until the age of 26 is reached.
Dental Insurance
Dental Insurance is provided by Guardian for full-time certified and classified employees. A full-time classified employee is scheduled to work 40 hours per week with the exception of cooks and bus drivers who shall be considered full-time if they are assigned to work 27.5 hours and 20 hours per week, respectively.

Part-time certified employees (working less than 7.75 hours per day) will be offered Dental Insurance at a prorated share based on the number of hours contracted per day.

To waive coverage, please complete and return the Enrollment form with the waiver section checked.

The dependent age limit is 19. If your dependent is a full-time student they can be covered until they reach the age of 26. The Student Certification Form must be completed to continue coverage. This form needs submitted to Guardian.
Life Insurance
Reynoldsburg City Schools Board of Education offers Life Insurance policies at no cost to full and part-time employees who work at least 50% of full-time hours.

Please list at least one beneficiary on the enrollment form.

Part-Time Classified Employees are offered $20,000 policies. Full-Time Classified and all Certified Employees are offered $40,000 policies. Administrators are offered $75,000 policies.
